Simultaneous appearance of leukemoid reaction and phlegmasia cerulea dolens

A leukemoid reaction is an extreme form of reactive leukocytosis defined as granulocytic leukocytosis above 50 × 109/L produced by normal bone marrow, mostly in response to systemic infection or cancer. The mechanism as to how the haematopoetic system is altered to elevate production of myeloid cells is not known. A 69-year-old man presented with phlegmasia cerulea dolens caused by massive iliofemoral thrombosis. His workout at admission revealed absolute white blood cell count of 73.4 × 109/L, with neutrophil granulocyte of 68.5 × 109/L. Flegmasia cerulea dolens en paciente con carcinoma testicular

Describimos un caso de flegmasia cerulea dolens secundaria a una trombosis venosa profunda por compresión de la vena cava inferior, en un varón de 31 años con un tumor germinal. Necesitó un tratamiento urgente con agentes fibrinolíticos, heparina intravenosa y quimioterapia. Con el tratamiento obtuvo una respuesta completa tumoral y una completa revascularización de la vena cava y vena femoral derecha.

Endovascular mechanical thrombectomy of the inferior vena cava and iliac veins with the use of Aspirex®S device in a paediatric patient

Phlegmasia cerulea dolens, which is the most severe clinical presentation of deep venous thrombosis and results from an almost complete occlusion of the major and collateral venous outflow routes from the extremity, is very rarely seen in children. Here we describe the treatment of an 11-year-old boy with Down syndrome who presented with thrombotic occlusion of the inferior vena cava and both iliac veins. We present a step-by-step technique of endovascular mechanical thrombectomy of these veins with the use of the Aspirex®S thrombectomy device.
